Box 10d is used to report the appropriate Claim Code or the Condition Codes whenever required and Box 19 is used to identify additional information about the patient’s condition or the claim.

This information can be entered into a claim from the Edit Charges/ Charge Master screen.
Pull up the required charge in charge master, click on the Detail Tab and then go to Others(F8). Now enter the details in the fields provided and then click on Save Enc.
